The last five years have seen remarkable improvements in several health indicators in West Bengal - infant and child mortality rates have fallen, more babies have been born in a health institution, there have been higher rates of child immunization, and people have had more access to maternal and child health services. As the Government of West Bengal’s programme to support its health sector strategy and strengthen the health scenario in the state comes to a close (Health Systems Development Initiative, HSDI: 2005-09), more and more people have benefitted from improved health services in the state over the last five years.

A workshop was held in Kolkata from 22-23 October 2009 to commemorate the achievements of the HSDI, share best practices and lessons learnt, and chalk out a path to take the process of health reforms forward. Minister in Charge, Department of Health and Family Welfare (DoHFW), Dr. Surja Kanta Mishra, Minister of State, DoHFW, S.K. Anwarul Haque, and Principal Secretary, Department of Women and Child Development, Ms. Rinchen Tempo, attended the workshop along with representatives from the UK Government’s Department for International Development (DFID) and dignitaries from within and outside the state.
Minister in Charge, Dr. Surja Kanta Mishra, released a booklet on the occasion, ‘Health Systems Development Initiative: Reforms and Achievements’. He also acknowledged DFID’s funding support that led to speedier health reforms in the state and improved numbers for maternal and infant mortality (a 27% drop in maternal mortality, one of the highest in the country – down 141 per 100,000 live births from 194 between 2001-03). These reforms, amongst others, have placed West Bengal amongst the ranks of better performing states in the country with the fourth lowest birth rate after Kerala, Tamil Nadu, and Punjab; third lowest death rate, after Delhi and Jammu and Kashmir; third lowest infant mortality rate, same as Delhi and after Kerala, Mahrashtra, and Tamil Nadu.
However, no reform process comes without its own bottlenecks and challenges. Some of these in West Bengal are:
- lack of health system monitoring at the district level and below
- physical distance to a qualified health institution, higher out-of-pocket expenses, and poverty are barriers to institutional deliveries
- inadequate regulation of the private sector
- large amount of unutilized funds, and
- shortage of manpower, especially doctors, across different health facilities in the state.

- 50% reduction in diseases from maternal, neonatal, & nutritional deficiencies

Technical Assistance Team – a partner for health reforms in West Bengal
A Technical Assistance Support Team (TAST) was engaged by DFID on behalf of the Government of West Bengal to support and strengthen health reforms in the state. Over the last five years, TAST complemented DFID’s budget support to the Government of West Bengal, accelerated reforms, and developed close working ties with the Department of Health and Family Welfare. A mid-term review of the Health Systems Development Initiative notes, “TAST in West Bengal has become integrated with the main system and has been recognized as a useful partner at all levels of state machinery”.
